La periodontitis reduce la capacidad fagocítica y de muerte intracelular de especies periodontopatogénicas del surco gingival. El uso de antibióticos como la clindamicina aumenta esta capacidad, mientras que el metronidazol y la doxiciclina la disminuyen. |

Resumen
Se determinó la fagocitosis in vitro y la muerte intracelular de Porphyromonas gingivalis ATC 33277 y de Actinobacillus actinomycetyemcomitans FDC 44 por polimorfonucleares (PMN) del surco gingival en la periodontitis severa. Se incluyeron 18 pacientes con periodontitis y 9 controles sanos. La fagocitosis y la muerte celular se comprobó mediante microscopia de fluorescencia luego de teñir con naranja de acridina. El porcentaje de PMN fagocitantes se estimó para dos grupos: <10 bacterias y >10 bacterias fagocitadas. También se registró el porcentaje de PMN con bacterias viables. En sujetos sanos se encontraron más fagocitos con mas de 10 P. gingivalis ingeridas que en los pacientes con periodontitis. La muerte intracelular disminuyó en el grupo de periodontitis para P. gingivalis y A. actinomycetemcomitans. Además, los efectos de concentraciones subinhibitorias de antibióticos (clindamicina, doxiciclina y metronidazol) se evaluaron por el mismo método. Cada granulocito se obtuvo de 13 - 24 pacientes con periodontitis y de 8 - 13 controles sanos. La clindamicina aumentó la capacidad de fagocitosis, al contrario de la doxiciclina y el metronidazol, que la disminuyeron. Estos efectos fueron independientes de la cepa utilizada. La muerte celular también pudo ser alterada por los antibióticos. Los resultados indican una disminución en la fagocitosis de especies periodontopatogénicas en la periodontitis y una influencia de los antibióticos utilizados en el tratamiento de esta respuesta inmune inespecífica.

PHAGOCYTOSIS OF PERIODONTOPATHOGENIC BACTERIA BY CREVICULAR GRANULOCYTES IN SEVERE PERIODONTITIS AND MODULATING BY ANTIBIOTICS
Abstract
In vitro-phagocytosis and intracellular killing of Porphyromonas gingivalis ATCC 33277 and of Actinobacillus actinomycetemcomitans FDC 44 by crevicular PMNs were determined in severe periodontitis. Each 18 patients with periodontitis and 9 healthy controls were included. Phagocytosis and intracellular killing were assessed by fluorescence microscopy after staining with acridine orange. The percentage of phagocytosing PMNs were enumerated and then two groups were separated: < 10 bacteria, > 10 bacteria phagocyted. The percentage of PMNs with viable bacteria was also registered. In healthy subjects there were more phagocytes with more than 10 ingested P. gingivalis than in periodontitis patients. The intracellular killing was diminished in the periodontitis group for P. gingivalis and for A. actinomycetemcomitans. Furthermore effects of subinhibitoric concentrations of antibiotics (clindamycin, doxycylin and metronidazole) were tested by the same method. Each granulocytes obtained from 13 - 24 periodontitis patients and 8 - 13 heathy controls were tested. Clindamycin enhanced phagocytosing capacity, contrary both doxycyclin and metronidazole decreased it. These effects were independent of the strain used. Also intracellular killing might be deviated by antibiotics. The results indicate a diminished phagocytosis of periodontopathognic species in periodontitis and an influence of antibiotics used in treatment on this unspecific immune response.
