O tratamento contribui para a descoberta de novas vias mais eficazes de administração de vacinas para a leishmaniose

Demonstramos que a via intranasal é eficaz para administração de vacinas normalmente exacerbadoras da doença quando administradas por via parenteral, como o antígeno bruto de Leishmania amazonensis e DNA que codifica a proteina LACK. O mecanismo de proteção parece envolver tanto tolerância periférica mas também imunidade ativa mista do tipo TH1 e TH2.

Nós demonstramos anteriormente que a administração oral de um antígeno particulado de Leishmania amazonensis promotor de doença (LaAg) parcialmente protege camundongos contra a leishmaniose cutânea. Neste trabalho, nós procuramos otimizar a vacina de mucosa usando a via intranasal para a administração de diferentes antígenos, incluindo o antígeno bruto LaAg, uma proteina recombinante de leishmania (LACK) e DNA codificando LACK (LACK DNA). Camundongos BALB/c pré-vacinados com duas doses de 10 microgramas de LaAg desenvolveram um controle tardio mas efetivo contra a doença. A diminuição da carga parasitária foi acompanhada de aumento da produção tanto de IFN-gama como de IL-10 nos linfonodos drenantes da lesão. A eficácia da vacina foi de longa duração, sendo ainda mais eficaz após 4 meses que após 1 semana da administração. Ao contrário da vacina bruta, a vacina recombinante administrada sem adjuvantes não foi eficaz pela via intranasal. Por outro lado, o LACK DNA (30 microgramas /dose) promoveu uma imunidade protetora rápida e duradoura. Após a infecção, o crescimento dos parasitos foi irrisório, e 5 meses depois células reativas ao LACK tanto nos linfonodos que drenavam a mucosa nasal como os que drenavam a lesão produziam altos níveis de interferon-gama. Estes resultados demonstraram pela primeira vez a possibilidade de se usar a via intranasal para induzir uma imunidade de longa duração contra a leishmaniose cutânea com antígeno bruto ou DNA na ausência de adjuvantes.
