METAS DE PRESION ARTERIAL EN PACIENTES CON DIABETES TIPO 2 TRATADOS CON INSULINA

La diabetes mellitus tipo 2 y la hipertensión arterial son 2 entidades que tienen una estrecha relación fisiopatológica, clínica y epidemiológica. En pacientes insulinizados, la prevalencia de hipertensión arterial duplica las tasas de la población general. En ellos, la meta terapéutica debería ser alcanzar cifras inferiores a 140/90 mmHg.

Resumen
La diabetes mellitus tipo 2 (DBT2) y la hipertensión arterial son dos entidades que tienen una estrecha relación fisiopatológica, clínica y epidemiológica. Si bien las normas de práctica clínica han incorporado el concepto de que los diabéticos podrían presentar una meta de presión arterial más baja que la población general, investigaciones recientes como el estudio ACCORD han establecido una duda al respecto. Se analizaron pacientes incorporados en el PRODIABA en el grupo de DBT2 que requirieron insulina y, con el objetivo de determinar la meta ideal de presión arterial, se estudiaron los predictores de mortalidad tomando el antecedente de hipertensión arterial y la presión arterial al ingreso, considerando para el análisis dos niveles comparativos: un primer nivel > 140/90 mm Hg y un segundo nivel > 130/80 mm Hg. Se tuvieron en cuenta como factores covariables edad, sexo, nivel de triglicéridos, colesterol y tabaquismo. De acuerdo con el modelo multivariado de regresión logística, el umbral por encima de 140/90 y el antecedente de hipertensión arterial al momento del ingreso al programa resultaron predictores de mortalidad. En pacientes con DBT2 insulinizados, la prevalencia de hipertensión arterial duplica las tasas de la población general. En ellos, la meta terapéutica debería ser alcanzar cifras inferiores a 140/90 mmHg. Los resultados del presente registro deberían justificar evaluaciones prospectivas en pacientes bajo tratamiento con hipoglucemiantes, para aclararlos objetivos ideales de presión arterial.

Abstract
Diabetes mellitus type 2 (DBT2) and hypertension are two entities that are closely related since pathophysiological, clinical and epidemiological view. Although clinical practice guidelines have incorporated the concept that diabetics could present a target blood pressure lower than the general population, since recent researches such as the ACCORD study has established a doubt. Were analyzed in patients enrolled in the PRODIABA Program DBT2 who had required insulin; in order to determine the goal of the blood pressure were analyzed the predictors of mortality: the history of hypertension, blood pressure at admission, considering for the comparative analysis of two levels: a first level > 140/90mm Hg, and a second level> 130/80 mm Hg. Age, sex, triglycerides, cholesterol and smoking were considered as covariates. According to multivariate logistic regression model, the cutoff level above 140/90 and a history of hypertension at entry to the program were the predictors of mortality. In patients with DBT2 insulinized the hypertension prevalence rates was twice than the general population. In these patients the therapeutic goal should be to achieve levels below 140/90 mmHg. The results of this record should justify prospective evaluations in patients treated with hypoglycemic, to clarify the ideal goals of blood pressure.
