Resumen
Introducción: El modelo matemático predictivo de mortalidad para la hemorragia supratentorial espontánea incorpora tres variables tomográficas (volumen del hematoma, desviación de la línea media y presencia de volcado ventricular) que mostraron predecir con alta sensibilidad y especificidad la evolución a los treinta días de los pacientes estudiados; sin embargo, este modelo aún no ha sido validado externamente. Objetivo: El propósito del presente estudio fue determinar si mediante este modelo se pueden estratificar pacientes de una cohorte independiente. Materiales y métodos: Se incluyeron en el presente estudio los pacientes que presentaron hemorragia supratentorial espontánea internados en la unidad de cuidados intensivos del Hospital Zonal de Agudos General Manuel Belgrano y de la Clínica Privada Independencia. Los criterios de ingreso al estudio, el análisis estadístico y la fórmula utilizada fueron las mismas que en el trabajo original, y se evaluó la mortalidad a los treinta días de producido el sangrado. Resultados: Se incluyeron en total 75 pacientes con una edad promedio de 53.8 años, el 80% correspondió al sexo masculino, la mortalidad global a los treinta días fue del 38.67%. De acuerdo con la prueba de bondad de ajuste, el modelo coincide con la realidad. La especificidad y la sensibilidad obtenidas en la cohorte analizada fueron del 93.5% y del 86.2%, respectivamente, y clasificó correctamente el 90.7% de los pacientes. El área bajo la curva ROC fue de 0.93. Conclusiones: De acuerdo con los datos obtenidos en el presente estudio podemos concluir que el modelo matemático analizado podría ser utilizado para estandarizar protocolos.

Abstract
Introduction: The predictive mathematical model of spontaneous supratentorial hemorrhage mortality incorporates three tomographic variables (hematoma volume, deviation from the middle line and presence of intraventricular hemorrhage) which were able to predict with high sensitivity and specificity the 30-day outcome of the patients studied. However, this model has not yet been fully externally validated. Objective: This study was conducted to determine if, by using this model, a patient stratification from an external cohort is possible. Methods: Patients admitted to the critical care unit of the "Hospital Zonal General de Agudos General Manuel Belgrano" and the "Clínica Privada Independencia" (Buenos Aires, Argentina) with supratentorial spontaneous hemorrhage were reviewed. Criteria for entry into the study, statistical analysis and the formula used were the same as in the original work, and mortality was assessed at 30 days from bleeding. Results: Seventy-five (75) patients were included. The average age of patients was 53.8 years, and 80% were male. Global 30d-mortality was 38.67%. According to the test of goodness-of-fit, the model matches reality. Specificity and sensitivity in the cohort analyzed were 93.5% and 86.2%, correctly classifying 90.7% of the patients. The area under the receiver operating characteristic curve was 0.93. Conclusions: According to the data obtained in the present study we can conclude that the mathematical model analyzed could be used to standardize protocols.
