NEFROPATIAS RELACIONADAS CON EL COLAGENO IV

La identificación de las mutaciones de los genes que codifican el colágeno IV tiene importantes consecuencias en el diagnóstico, pronóstico y seguimiento de las enfermedades relacionadas.

Resumen
Se ha demostrado que las proteínas para las que codifican los genes COL4A3, COL4A4 y COL4A5 están implicadas en distintas manifestaciones renales. Mutaciones en estos genes del colágeno IV afectan la estructura de la membrana basal glomerular dando lugar a una nefropatía cuyos síntomas oscilan desde la hematuria aislada hasta la insuficiencia renal. Estas alteraciones renales han sido consideradas históricamente como distintas entidades: síndrome de Alport ligado al sexo, síndrome de Alport autosómico recesivo, síndrome de Alport autosómico dominante, hematuria familiar benigna y portadores del síndrome de Alport autosómico recesivo. Pero el conocimiento molecular de estas enfermedades ha hecho que podamos agruparlas bajo otros epígrafes, como son: síndrome de Alport ligado al sexo, síndrome de Alport autosómico recesivo y nefropatía del colágeno IV. Este hecho tiene importantes consecuencias en el diagnóstico, pronóstico y seguimiento de la enfermedad.

Abstract
It has been demonstrated that the proteins codified by the COL4A3, COL4A4 and COL4A5 genes, are linked to different renal manifestations. Mutations in collagen IV genes affect the structure of the glomerular basal membrane giving rise to a nephropathy whose symptoms vary from isolated haematuria to renal insufficiency. These renal manifestations were historically considered different diseases: X linked Alport syndrome, autosomal recessive Alport syndrome, autosomal dominant Alport syndrome, familial benign haematuria and carriers of autosomal recessive Alport syndrome. Molecular knowledge has made possible the renaming of these diseases as X linked Alport syndrome, autosomal recessive Alport syndrome and collagen IV nephropathy. This has important implications regarding the diagnosis, prognosis and long-term follow up in these diseases.
