Resumen
La fracción de eyección del ventrículo izquierdo (FEVI) es un índice de gran utilidad en la estratificación de riesgo en pacientes con miocardiopatía isquémica o primaria. Con el gated SPECT se pueden calcular con cierta precisión los volúmenes ventriculares y la FEVI. Varios estudios clínicos han demostrado la eficacia del gated SPECT en el cálculo de este parámetro. Sin embargo existen algunos factores que disminuyen la efectividad del gated SPECT y deben ser considerados por el clínico o los investigadores que utilizan este método para medir la FEVI. Estos factores son: la actividad de fondo, el número de cuentas radiactivas en la imagen, el tamaño del ventrículo izquierdo y la presencia de arritmias y/o defectos de perfusión. Finalmente, está demostrado que la variabilidad intrínseca del gated SPECT es alta y por tanto no debe ser utilizado en análisis secuenciales de la FEVI. Sobre la base de estos conocimientos, cuando es necesaria la evaluación precisa y secuencial de la FEVI, la ventriculografía radioisotópica en equilibrio (VRIE) continúa siendo el método más confiable para evaluar la fracción de eyección del ventrículo izquierdo.

Abstract
Left ventricular ejection fraction (LVEF) is a major predictor of prognosis in patients with ischemic and non-ischemic heart disease. Gated SPECT has been used for computation of left ventricular volumes and ejection fraction and it provides a highly reproducible estimate of LVEF. Although the accuracy of gated SPECT techniques for estimation of LVEF has been reasonable in several clinical studies, physicians and investigators need to consider some potentially confounding issues when reviewing the results of quantitative gated SPECT analysis. The accuracy of gated SPECT might be affected by background activity, counting statistics, left ventricular size, arrythmias, and abnormal perfusion images. Furthermore, intrinsic variability of sequential LVEF estimations, using gated SPECT, is substantial and should not be used for serial evaluation of LVEF. In light of this, radionuclide evaluation of LVEF by gated SPECT does not have the same accuracy as equilibrium radionuclide angiography (ERNA). ERNA measurements have been shown to be reliable for the detection of relatively small changes in LVEF, which allows the physician to make management decisions on the basis of precise values of LVEF.
