BTA-TRAK URINARIO EN EL SEGUIMIENTO DEL CARCINOMA TRANSICIONAL DE VEJIGA SUPERFICIAL.

Resumen
Hemos estudiado 700 determinaciones consecutivas de BTA-TRAK realizadas durante el seguimiento de pacientes con carcinoma transicional de vejiga. De todas ellas, 95 (13.6%) correspondieron a carcinoma urotelial descubierto durante el seguimiento, de vejiga (93) o de tracto urinario superior (2). La sensibilidad de la citología fue del 41.1%, con especificidad del 97.3%. La sensibilidad del sedimento para detectar tumor urotelial (microhematuria) fue del 40%, con especificidad del 96.7%. Tomando el valor de referencia de normalidad de BTA-TRAK propuesto (≤ 14 U/ml), hemos encontrado sensibilidad del 62.1% y especificidad del 68.4%. Se elaboró un modelo de regresión logística incluyendo BTA-TRAK, sedimento y citología urinaria, para conocer las variables independientes útiles en la detección de carcinoma vesical superficial en nuestra serie. Cuando se combinaron los 3 parámetros, la citología tuvo odds ratio (OR) = 18.5 (8.9-38.5) y el sedimento OR = 11.8 (5.9-23.5), quedando el BTA-TRAK fuera de la ecuación. Conclusión. Aunque la sensibilidad global del BTA-TRAK es superior a la de la citología y a la del sedimento, no aporta información adicional a la de la combinación de citología + sedimento en la detección de recidivas tumorales de carcinoma superficial de vejiga.

Abstract
700 consecutive determinations using BTA-TRAK were performed to monitor unselected patients with transitional cell carcinoma of the urinary bladder. Of the 700 determinations, 95 (13.6%) were urothelial carcinomas (93 bladder, 2 upper urinary tract). The sensitivity of urine cytology to detect urothelial tumor was 41.1% and the specificity was 97.3%. The sensitivity of urine analysis to detect urothelial tumor was 40% and the specificity was 96.7%. Considering the proposed normal reference value for BTATRAK (≤ 14 U/ml), we have found a sensitivity of 62.1%and a specificity of 68.4%. A logistic regression model was developed including BTATRAK, urinary analysis and cytology, to identify the independent variables that are useful for tumor detection during follow-up of superficial carcinoma of the bladder in our series. The combination of three variables showed an odds ratio of 18.5 (8.9-38.5) for urine cytology, 11.8 (5.9-23.5) for urine analysis and an odds ratio for BTATRAK that did not fall within the equation. Conclusion. Although overall the sensitivity of BTATRAK is higher than that of urine cytology and urine analysis, it provides no additional information than that obtained from the combination of urine cytology and urine analysis in the detection of tumor recurrence during monitoring for superficial bladder cancer.
