Abstract
Objective. 1) To evaluate the frequency of chronic anterior uveitis in patients with juvenile rheumatoid arthritis (JRA) and its association with the presence of antinuclear antibodies (AAN). and 2) to evaluate the frequency of auto-immune uveitis among the uveitis of unknown origin.Patients and methods. We studied retrospectively 76 JIA patients. All of them were submitted to slit-lamp examination of the anterior chamber at the diagnosis. AAN was performed at the disease onset. Besides this, 29 patients with idiopathic uveitis were evaluated in relation to the AAN positivity and follow-up.Results. Chronic anterior uveitis was detected in 9 children (11.8%). Among them, 7 (77.8%) had pauciarticular AIJ at disease onset and 2 girls with polyarticular. In this group the mean age at JIA onset was 4.6 years old and the mean age of uveitis onset was 6.5 years old. AAN was positive in 6/9 patients (67%) with uveitis. Five children had 2 or more flares of uveitis. Concerning the complications, 4 patients developed cataract (one with associated glaucoma), one posterior synechiae and one decreased visual acuity. The frequency of AAN was statistically higher in the patients with uveitis (67%) than in those without uveitis (12%) (p<0.05). Of the 29 patients with idiopathic uveitis (9 with acute uveitis and 20 with chronic uveitis), four (13.8%) presented recurrences, all with AAN positivity. Among the 25 patients with idiopathic uveitis without recurrence, all but three were AAN negative.Conclusion. Although the incidence of uveitis in our study was lower than the literature, the frequency of uveitis was higher in the girls, in the pauciarticular group and in the AAN positive patients.
