ESTUDIOS GENETICOS EN PACIENTES MEXICANOS CON ESPONDILOARTROPATIAS

Nuestros estudios establecen que además del HLA-B27 existen otros genes que pueden participar en la susceptibilidad genética al desarrollo de las espondiloartropatías. Estos genes podrían también condicionar las manifestaciones clínicas de estos padecimientos.

Resumen
Las espondiloartropatías (SpA) son un grupo de enfermedades asociadas con el antígeno de histocompatibilidad HLA-B27. Noventa por ciento de los pacientes con espondilitis anquilosante son positivos para el HLA-B27, sin embargo, únicamente 3% a 5% de los individuos B27 positivos desarrollan la enfermedad, lo que sugiere que otros marcadores genéticos se asocian con la susceptibilidad a las SpA. Con el fin de detectar otros marcadores genéticos asociados con SpA, nosotros estudiamos varios polimorfismos en un grupo de pacientes mexicanos con diagnóstico de SpA (espondilitis anquilosante, artritis reactiva y espondiloartropatías indiferenciadas). Se aplicaron a los pacientes cuestionarios de actividad y funcionalidad (BASFI y BASDAI). Nuestros datos corroboran la fuerte asociación de las SpA con el HLA-B27 pero también detectamos otros marcadores asociados (HLA-B15 y HLA-DR1). Se observó que los marcadores HLA tienen efecto en la edad de inicio y en la gravedad de la enfermedad. Pacientes B27 positivos inician a edades más tempranas y su enfermedad es más grave que la que desarrollan los pacientes B27 negativos. Además, los pacientes HLA-DR1 positivos inician a edades más tardías que los pacientes DR1 negativos. Los genes de las proteínas de choque térmico (HSP) y los del proteosoma (LMP) únicamente mostraron asociación con las formas indiferenciadas de la enfermedad.

Abstract
The spondyloarthropathies (SpA) are a group of diseases associated with the B27 histocompatibility antigen. Ninety percent of the ankylosing spondylitis patients are B27 positive, however, only 3-5% of the B27 positive individuals develope the disease, suggesting that other genetic markers are associated with the susceptibility to SpA. In order to detect other genetic markers associated with SpA, we studied several polymorphisms in a group of Mexican patients with diagnosis of SpA (ankylosing spondylitis, reactive arthritis and undifferentiated spondyloarthropathies). Questionnaires of activity and functionality (BASDAI and BASFI) were applied to the patients. Our results corroborate the strong association of SpA with HLA-B27, but we also detected other associated markers (HLA-B15 and HLA-DR1). We observed that the HLA markers have effect on the age at onset and in the severity of the disease. B27 positive patients had a lower age at onset than B27 negative individuals and the disease was also more severe. In addition, the HLA-DR1 positive patients had an age at onset higher than DR1 negative patients. Other genes such as the heat shock protein (HSP) and large molecular weight proteosome (LMP) only showed association with the undifferentiated forms of the disease.
