Revisión de la evidencia existente en cuanto a la reperfusión farmacológica o mecánica en el infarto agudo de miocardio que ocurre en pacientes ancianos. Valoración de los riesgos y limtaciones de la fibrinólisis y de la angioplastia primaria, así como análisis de sus beneficios.

Resumen
La prevalencia de la cardiopatía isquémica aumenta con edad. La presentación clínica atípica, la comorbilidad y la técnica de reperfusión, si la hay, conllevan un retraso en el tratamiento del paciente anciano. La angioplastia primaria en el manejo del infarto agudo de miocardio ha demostrado claramente su beneficio sobre la fibrinólisis en pacientes jóvenes pero no está tan claro que abordaje seguir en los ancianos. Muchos son los pacientes mayores que son candidatos a un tratamiento de reperfusión, pero presentan contraindicaciones a la fibrinólisis. La alta incidencia de complicaciones de la fibrinólisis hace que, en la práctica diaria, no sea una técnica de reperfusión usada ampliamente en este subgrupo. Aunque hay indicios de que la angioplastia pueda ser también superior en esta franja etaria, los estudios realizados hasta el momento (Zwolle, SENIOR PAMI y TRIANA) muestran resultados dispares. Al realizar un metanálisis de los 3 estudios, se extrae información confiable de que la angioplastia primaria es superior a la fibrinólisis, con una reducción significativa de mortalidad, reinfarto e ictus incapacitante. Aunque la angioplastia primaria es la estrategia de elección en el paciente anciano, la fibrinólisis debe considerarse como una alternativa cuando la angioplastia primaria no esté disponible.

Abstract
The prevalence of ischemic heart disease increases with age. The atypical clinical presentation, comorbidity and reperfusion technique involve a delay in management of elderly patients. In the management of acute myocardial infarction in young patients, primary angioplasty has clearly demonstrated its benefits over fibrinolysis, but it is not so clear which treatment to follow in the elderly. Many older patients are candidates for reperfusion therapy but have contraindications to fibrinolysis. This technique is not widely used in this subgroup of patients on a regular basis given the high incidence of complications. While there are indications that angioplasty may also be more suitable in this age range, the studies conducted so far (Zwolle, SENIOR PAMI and TRIANA) show mixed results. When performing a meta-analysis of the 3 studies, important evidence has been extracted to support the fact that primary angioplasty is superior to fibrinolysis with a significant reduction of death, reinfarction and disabling stroke. Although primary angioplasty is the strategy of choice in elderly patients, fibrinolysis should be considered as an alternative when primary angioplasty is not available.
