CIRUGIA RADICAL EN CANCER CERVICOUTERINO PERSISTENTE O RECURRENTE A RADIACION: EXPERIENCIA CON 500 PACIENTES

Se muestra una experiencia institucional en el tratamiento quirúrgico del cáncer cervicouterino recurrente a radiación, con más de 900 pacientes, refiriendo los procedimientos llevados a cabo: laparotomías y bioposia, histerectomías radicales y exenteraciones pelvianas. Se analiza además la morbimortalidad de la cirugía radical y los factores pronósticos de las exenteraciones pelvianas.

Resumen
Introducción: Las laparotomías destinadas a realizar algún tipo de cirugía de rescate constituyen la última oportunidad para la curación de un grupo seleccionado de pacientes con cáncer cervicouterino recurrente a radiación. Objetivo: Mostrar una experiencia institucional de 40 años en pacientes con cáncer cervicouterino, exploradas quirúrgicamente con el fin de realizar una exenteración pelviana o una histerectomía radical clase 3. Material y métodos: Revisión de la casuística del Servicio de Oncología del Hospital General de México entre los años 1966 y 2006. Resultados: Se realizaron 931 laparotomías con 429 exenteraciones pelvianas y 71 histerectomías radicales. Cuatrocientas treinta y un pacientes (46.2%) mostraron lesiones irresecables, de las cuales el 73.3% tenía tumor fuera de la pelvis. Se efectuaron 71 histerectomías radicales, de las cuales el 28.1% presentaron complicaciones graves. Se produjeron 4 fallecimientos (5.6%) y el 61.2% de las participantes evolucionó, a los 30 meses, sin evidencia de enfermedad. Se realizaron 214 exenteraciones anteriores (49.8%), 208 totales (48.4%) y 7 posteriores (1.6%). El 30% presentó complicaciones graves, con una mortalidad operatoria global del 10.7%, la cual descendió al 3.7% en los últimos 6 años del estudio. El 48.8% de estas pacientes tuvo un seguimiento a 24 meses sin evidencia de enfermedad. Se incluye el 58.3% de las exenteraciones anteriores y el 41.7% de las exenteraciones totales (p = 0.04). Conclusiones: La cirugía radical, representada por las exenteraciones pelvianas, mantiene su vigencia en el tratamiento del cáncer cervicouterino persistente o recurrente a radiación.

Abstract
Background: Laparotomies as a kind of rescue surgery represent the last chance of cure in a selected group with cervical cancer recurrent after radiotherapy. Objective: To show a 40-year institutional experience of laparotomies in patients with cervical cancer in an attempt to perform a pelvic exenteration or a class 3 hysterectomy. Material and methods: A review of cases at the Oncology Service of the Hospital General de México from 1966 to 2006. Results: Nine hundred and thirty-one laparotomies were conducted with 429 pelvic exenterations and 71 radical hysterectomies. Four hundred and thirty-one patients (46.2%) were irresectable, 73.3% of whom had a tumor outside the pelvis. Seventy-one radical hysterectomies were made and 28.1% developed major complications. Four patients died (5.6%) and 61.2% were free of disease at 30 months. Two hundred and fourteen anterior exenterations were made (49.8%), 208 total exenterations (48.4%) and 7 posterior exenterations (1.6%). Thirty percent developed major complications with a global operative mortality of 10.7% which was reduced to 3.7% in the last 6 years. Forty-eight percent of these patients were free of disease at 24 months. A total of 58.3% of the anterior exenterations and 41.7% of the total exenterations (p = 0.04) were included in this study. Conclusion: Pelvic exenteration is an elegible surgical procedure for persistent or recurrent cervical cancer after radiation therapy.
