Resumen
El manejo inicial de la angina instable y del infarto de miocardio sin elevación del segmento ST pone especial énfasis en la selección de una estrategia invasiva temprana o de una estrategia conservadora. La estrategia invasiva temprana se recomienda actualmente para los pacientes con angina inestable e infarto de miocardio sin elevación del segmento ST con riesgo clínico moderado y alto. La controversia sobre si en los pacientes con síndromes coronarios agudos debe emplearse una estrategia invasiva temprana o una estrategia conservadora surgió a partir de los conflictivos resultados de los primeros estudios clínicos (TIMI-IIIB, MATE, VANQWISH) y del registro internacional OASIS. Sin embargo, los estudios clínicos contemporáneos (FRISC-II, TRUCS, TACTICS TIMI-18, VINO, RITA-3) demostraron consistentemente el beneficio del empleo de la estrategia invasiva temprana en el punto final combinado de angina refractaria, infarto y muerte, aunque la mortalidad como punto final aislado no se ha logrado abatir con la estrategia invasiva. La evidencia actual sugiere que el uso de la estrategia invasiva temprana junto con una terapéutica antitrombótica óptima (anticoagulante y antiplaquetaria) debería ser de elección.

Abstract
The initial management of unstable angina and non ST segment elevation myocardial infarction deserve special emphasis regarding the choice of an early invasive or conservative strategy. An early invasive strategy is currently recommended for patients with unstable angina and non ST segment elevation myocardial infarction with moderate to high risk features. Whether patients with acute coronary syndromes should undergo an early invasive or conservative management strategy had been controversial because the conflicting results of the early clinical trials (TIMI-IIIB, MATE, VANQWISH).and the OASIS multinational registry. However contemporary recently completed trials (FRISC-II, TRUCS TACTICS TIMI-18, VINO, RITA-3) have consistently shown benefit using an early invasive strategy on the combined outcome of refractory angina, myocardial infarction, and death, although the mortality as an isolated end point did not change. Current evidence suggests that the use of an invasive approach together with an aggressive anticoagulation and antiplatelet therapy should be the choice.
