TRATAMIENTO DEL ASMA BRONQUIAL CON BETAMETASONA POR VIA SUBCUTANEA EN NIÑOS. COHORTE DE 12 AÑOS

Los niños con asma moderada y grave tratados con betametasona subcutánea tienen menor riesgo de exacerbaciones de las crisis y de reacciones adversas a los glucocorticoides que los que reciben el tratamiento convencional.

Resumen
Antecedentes: Muchos pacientes con asma moderada y grave no responden al tratamiento convencional que incluye, entre otros medicamentos, diferentes ciclos de prednisona oral. Material y método: Se realizó una selección de 6 400 pacientes con asma moderada y grave de las edades comprendidas entre 2 y 14 años. Por asignación aleatoria, la mitad de ellos recibieron betametasona por vía subcutánea en dosis de 0.1 mg/kg/día durante 4 días sin exceder los 4 mg por dosis en los períodos de exacerbación de las crisis, la otra mitad recibió el tratamiento de consenso. En ambos grupos se valoró el número de crisis por año, ingresos hospitalarios, calidad de vida y reacciones adversas a los glucocorticoides. El seguimiento en ambos grupos se llevó a cabo durante 12 años. Resultados: Terminaron el estudio 5 400 pacientes, 2 810 del grupo de estudio y 2 690 del grupo control. El consumo de broncodilatadores presentó un riesgo relativo (RR) de 0.51; intervalo de confianza (IC) de 95%, 0.48-0.54. Los ingresos hospitalarios por año en el grupo de estudio con respecto al grupo control presentaron un RR de 0.54 (0.51-0.58); la calidad de vida en el grupo de estudio tuvo un RR de 0.50 (0.47-0.54). En cuanto a las reacciones adversas a los glucocorticoides, en el grupo de estudio con respecto al grupo control, los RR fueron, para el síndrome de Cushing, 0.51 (0.44-0.58); ganancia de peso, 0.47 (0.42-0.54); supresión del crecimiento lineal 0.71 (0.65-0.78); hipertensión arterial, 0.63 (0.56-0.72), y para adelgazamiento de la piel, 0.80 (0.74-0.88). Conclusiones: Los niños con asma moderada y grave tratados con betametasona subcutánea tienen menor riesgo de exacerbaciones de las crisis y de reacciones adversas a los glucocorticoides que los que reciben el tratamiento convencional.
